Coordinated Intake

Here in Oak Park and River Forest, we serve as the single point of contact for connecting families to home visiting programs and high quality preschool programs. In other words, we coordinate with a our partners to connect families to services so that they don’t have to. Coordinated intake allows families to receive services more quickly and it also ensures that those who are most eligible are receiving those services.

Families who believe they may be eligible for free preschool can email or call 708-613-6122 x 3 and our Director of Early Learning will talk with them to get a deeper understanding of their unique needs. From there, she will coordinate with the three agencies in Oak Park that provide state-funded Preschool for All programs: ABC Toon Town II Educational Center, Oak Park Elementary School District 97 and Oak Park River Forest Day Nursery to ensure the right fit.

For families who would like the support of home visiting services, or for those who would like to refer a family for these services, we work in partnership with the Village of Oak Park Public Health Nurse. Referrals and requests for support can be shared in a variety of ways:

The Home Visiting Task Force

The Collaboration’s Manager of Family Engagement facilitates the Home Visiting Task Force, comprised of a team of representatives from home visiting programs, who meet regularly to further streamline referral processes and share updates so that the coordinate intake process is up-to-date and efficient for families.
